.CHECKLIST FOR Tourist Visa PhillippinesS.No.documents attached1Passport: Original passport with minimum 6 months validity from the time of travel and minimum 2 empty pages for the visa.

2Visa Application Form: Fully completed and signed by the applicant. (empty/blank fields may be cause for invalid application).

3Photographs: 2 latest passport type 35mm x 45mm sized colour photos with white background. Scanned photographs are not acceptable.

4Ticket Itinerary (Both way flight tickets): Copy of return flight tickets with applicant name. If applying in a group, please highlight applicant name.

5Onward Country Visa: If applicant travelling to other countries rather than his/her country of residence/origin.

6Proof of Accommodation: Copy of hotel booking with applicant name or a letter from the host with host’s ID/passport copy.

7Employment Letter (Original):

If Employed: from the company in India with clear company details, applicant details, from how long he/she is working, a period of leave approved with Manager/HR seal & signature. and assurance applicant will back to work immediately after this trip.

If the Applicant is self-employed, a copy of the registration of his Business.

Important Notes:

1.Embassy may ask for more documents (if required).2.Indian nationals with a valid US, Japanese, Australian, Canadian, Schengen, Singapore, OR United Kingdom visa will be granted a 14-day visa-free entry which may be extended for an additional 7 days for a maximum stay of 21 days. It can not be converted to another visa category.3.If the Applicant is a minor, documents of the parent’s or legal guardian to show income and financial capacity for the last 2 years is required.4.For trips more than 21 days, Indian Nationals are required to apply for a tourist visa at the Embassy in New Delhi.
